The Office of Child Care of the Maryland State Department of Education
requires specific forms for children enrolled in Aftercare. The Health
Inventory and Medication Authorization require completion and signature
by the child’s health care provider. Please refer to the OCC MSDE
website below to assure you have current forms for the health care
provider to complete.

Emergency Information is OCC1214
Health Inventory is OCC1215
Both of these forms are required for each child enrolled in our Aftercare Program.
If your child requires medication during Aftercare hours (please
consider early dismissal days), authorization to administer is also
required. The Medication Authorization is OCC1216.
